Replacement Logbook

If your logbook has been lost, stolen or destroyed, you will need to apply for a replacement logbook. Please complete the following steps.

STATUTORY DECLARATION FORM

Complete a statutory declaration form. The declaration must include the relevant vehicle logbook number and provide a reason for requiring replacement.

LOGBOOK APPLICATION FORM AND PHOTOS

You must complete a NEW APPLICATION FORM and provide photos as per the above procedure.

FEE

The fee of $80.00 must be sent with the application. The application will be processed within 2 weeks (providing all paperwork is correct). If the logbook is required before that time a $70 priority fee applies making the total fee $150.00
